- If you choose not to set up an e-mail account you may enter a user name consisting of letters and numbers (no spaces). However, if you do not supply a valid e-mail address you must download or print your filled-in application before leaving this site.
- If you do not supply an e-mail address or a user name you will have to retype all of your information if you wish to use this site in the future to reapply for an FPCA.
